The Direct Support Professional – HSW provides individualized Supported Living Services (SLS) to adults with intellectual and/or developmental disabilities. Employees assigned to HSW caseloads may be required to provide specialized supports, complete advanced training requirements, demonstrate client-specific competencies, or meet HSW service expectations based on the needs of the individuals they support.
Support requirements vary by assignment. Employees must be able to adapt to individualized client needs and successfully implement support strategies, safety procedures, health-related supports, mobility assistance, behavioral supports, supervision requirements, and other client-specific responsibilities as trained and assigned.
The Direct Support Professional – HSW is expected to maintain professionalism, sound judgment, flexibility, and a commitment to providing safe, person-centered services in home and community settings.
- Provide direct support and supervision to clients in accordance with individualized support plans and assigned service requirements.
- Implement individualized support plans, safety plans, support strategies, health-related supports, mobility protocols, and other client-specific procedures as assigned.
- Support clients with activities of daily living, including meal preparation, feeding assistance, personal hygiene, dressing, toileting, household tasks, shopping, and community participation.
- Assist clients with mobility, transfers, repositioning, wheelchair support, gait-belt assistance, adaptive equipment, and other physical support needs as trained and assigned.
- Maintain supervision levels consistent with individualized support plans and client safety needs.
- Monitor for changes in client health, safety, behavior, emotional well-being, environmental conditions, or other factors that may impact service delivery or client welfare.
- Respond appropriately to emergencies, behavioral incidents, medical concerns, and crisis situations in accordance with company policy, training, and individualized support protocols.
- Utilize person-centered, trauma-informed, and respectful support approaches at all times.
- Encourage client independence, informed decision-making, self-advocacy, and participation in meaningful activities.
- Assist clients in accessing educational, vocational, recreational, social, and community-based opportunities.
- Maintain accurate and timely documentation, including progress notes, incident reports, medication documentation (if assigned and trained), and other required records.
- Communicate effectively with supervisors, family members, interdisciplinary team members, healthcare professionals, and other support persons as appropriate.
- Maintain confidentiality and professional boundaries at all times.
- Transport clients safely within the community as assigned.
- Participate in required meetings, trainings, competency reviews, and ongoing professional development.
