Location: Northern
UF Health Flagler Hospital invites applications for a Community Health Associate to deliver direct community health services through Care Connect+. You will engage vulnerable populations, assess needs, and connect clients with housing, health, and social support resources in a fast-paced environment.
Responsibilities include trauma-informed patient education, crisis response, and thorough documentation in the Care Connect+ system, with a caseload up to 150. Background screening is required.
