Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community.

As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants.

ROLE OVERVIEW and PURPOSE

The PACE Rehabilitation Manager oversees the operations of the rehabilitation department, ensuring high-quality services that align with state and federal regulations. This role provides administrative supervision and leadership for the therapy team, including physical therapy, occupational therapy, and speech language pathology services. Clinical supervision is provided within the manager's licensed scope of practice, while designated discipline-specific leads or discipline-specific evaluating therapists provide clinical oversight for services outside the manager's licensed discipline.

Schedule:
Monday-Friday 8am-4:30pm.

Responsibilities
  • Orients new staff to the PACE (Program of All-Inclusive Care for the Elderly) model, CMS, and regulatory requirements while ensuring compliance with PACE guidelines
  • Provides ongoing coaching, mentoring, and training to promote clinical excellence and team cohesion
  • Leads, coordinates, and supervises licensed therapy professionals within scope of practice (this may include disciplines of Occupational Therapy, Physical Therapy, and Speech Language Pathology) and aides, ensuring continuity and consistency of participant care planning
  • Monitors staffing patterns, caseload distribution, and service coverage to ensure operational efficiency and appropriate workload balance
  • Supports workforce planning, onboarding, and role development to meet evolving program needs
  • Maintains competency in Interdisciplinary Team (IDT) processes and facilitates IDT meetings as directed, ensuring effective coordination of care and operational alignment
  • Ensures standardization of operations within scope; identifies, reports, and addresses variances with rationale to maintain alignment with program expectations
  • Ensures clinical supervision structures are clearly defined and maintained across disciplines, with appropriate delegation and oversight in accordance with scope of practice and regulatory requirements
  • Assists in developing policies and procedures to ensure department best practices are followed, therapy interventions meet quality standards, and all activities are safe, culturally appropriate, and compliant with regulatory standards
  • Develops and implements processes to ensure departmental continuous tracking and monitoring of the provision of rehabilitative related services
  • Attends and participates in staff meetings, organizational QI activities, and assigned committees, including participant care planning, utilization reviews, ethics, grievances, and appeals
  • Oversees Home Health Occupational Therapy, Physical Therapy, and Speech Language Pathology provision of services ensuring quality and timely services for participants in their homes
  • Develop and manage the DME process, including ordering, storage, maintenance, and tracking, ensuring compliance with regulatory standards
  • Oversee DME safety and in-home installations to enhance participant independence and safety
  • Ensures delivery of quality services by overseeing the department's adherence to policies/procedures and compliance with state and federal regulatory requirements
  • Ensures services provided are in a therapeutic setting and conducive to restoring dignity and self-esteem to patients
  • Monitor site and department budgets to maximize cost-effectiveness of operations while ensuring high-quality care is maintained
