Job SummaryThe Intake Specialist is responsible for managing thepatientintake process, ensuring all necessary documentation, referrals, and eligibility requirements are gathered efficiently and accurately. The role involves direct interaction with patients, families, healthcare providers, and insurance companies to ensure that new patients are enrolled into homecare services in a smooth and compliant manner. The Intake Specialist plays a critical role infacilitatingtimelycare transitions for patients and ensuring the overall quality of the homecare experience from the start.
Essential Duties and Responsibilities:- Handle theinitialintake process for new patients, including gathering essential information (e.g., medical history, demographics, insurance details).
- Ensureaccurateentry of patient information into the company’s electronic medical record (EMR) system and other relevant databases.
- Verify clients' insurance coverage and eligibility for home care services, ensuringaccuratebilling and reimbursement.
- Answer incoming calls from clients, caregivers, and healthcare professionals, addressing inquiries and providing information about available services.
- Conduct comprehensiveinitialassessments to evaluate clients' healthcare needs anddeterminetheappropriate levelof ca rerequired.
- Collaborate with marketing representatives to stay informed of updates and changes, providing quick and relevant information to clients and stakeholders.
- Assist clients in accessing Certified Home Health Agency (CHHA), Managed Long-Term Care (MLTC), and Personal Care Aide (PCA) services.
- Educate clients on the eligibility criteria for home care services, explaining their options and helping them make informed decisions about their care.
- Serve as the primary point of contact for patients, families, and referral sources to answer questions and provide updatesregardingthe intake process.
- Maintain confidentiality and ensure compliance with HIPAA and other patient privacy laws.
- Escalate complex cases to the Intake Supervisor or Director of Intake for further resolution.

- Minimum of 1 year of experience in home care intake or enrollment processes.
- Exceptional communication and interpersonal skills, with the ability to effectively interact with clients and stakeholders.
- Highly organized, detail-oriented, and capable of managing multiple tasks efficiently.
- Proficiency in Microsoft Office applications and experience with HHA Exchange software.
- Understandingofhome care services, insurance verification processes, and enrollment procedures.
