Key Responsibilities

Intake and Discharge Responsibilities (30%)
  • Inputs client and program records into HMIS system.
  • Conducts discharge of clients according to program procedures.
Monitoring Responsibilities (60%)
  • Monitors daily activities and ensures client program participation; ensures the safety, cleanliness, and orderliness of program environment; ensures clients maintain appropriate attire and personal hygiene; observes clients for safety, health and risk potentials.
  • Communicates with members of the clients’ treatment team as may be requested for purposes of providing additional client information; communicates all major incidents to clinical supervisor and primary counselor in writing and orally; facilities ongoing information exchange.
  • Responds to emergencies in a calm manner and initiates appropriate corrective actions in accordance with established policies and procedures; provides necessary crisis intervention; ensures 24-hour monitoring of clients; informs Director of Social Services of special problems, needs, etc.
  • Inspects program area daily; provides individual instruction or correction to clients regarding proper use of the facility as needed; reports damage or other maintenance needs to Program Manager.
  • Periodically monitors bed assignments; wakes up clients at designated time and instructs them to collect their sheets; ensures that all residents are out of the facility by the designated time; distributes client luggage.
  • Assists in serving clients’ meals.
Administrative Responsibilities (10%)
  • Completes required paperwork representing client intake for the day; inputs data into the computer to maintain computer records and generate client lists.
  • Maintains client and program records daily; submits required reports and statistics; prepares written incident reports.
  • Performs data entry and light typing as required
WHAT WE ARE LOOKING FOR IN YOU

Education and Experience:
  • High school diploma or G.E.D. required, AND
  • One year experience performing security or social service work preferred, OR
  • Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
Certifications:
  • Valid State Driver’s License (Preferred)
  • Food Handlers Certification (or ability to obtain within established timeframe)
  • CPR and First Aid Certification (or ability to obtain within established timeframe)
