We are seeking a licensed and experienced Physical Therapist to provide outpatient rehabilitation services in an in-home setting for the older adult population. This includes working in private residences or independent living facilities.
Key Responsibilities- Assess, develop, and deliver physical therapy programs to support rehabilitation.
- Identify the need for referrals to other disciplines as required for patient care.
- Implement individualized therapy plans to restore, reinforce, and enhance physical performance.
- Educate and train patients and caregivers on essential skills to promote independence and productivity.
- Direct health-promotion activities to maintain overall well-being.

Qualifications- Valid Physical Therapy license in the applicable state(s) or proof of eligibility.
- Master's degree in Physical Therapy (or a related field) or a Bachelor's degree in Physical Therapy. Degrees earned outside the U.S. must be evaluated for equivalency.
- Minimum of 5 years of progressive experience in Physical Therapy.
- Valid driver's license or reliable transportation for in-home patient visits.
- Ability to meet physical demands, including lifting 50 lbs and standing or walking for extended periods.
- Compliance with legal standards and policies, including HIPAA and credentialing requirements.
