Person Centered Services Care Coordination Organization is seeking Care Coordinators in a hybrid role to support individuals under a health home model. Candidates will coordinate health, behavioral health, and community supports, advocating for person-centered care with a strengths-based approach.
Minimum qualifications include a Bachelor’s degree or RN license with relevant experience;
Master’s degree with 1 year of experience is also acceptable. Valid driver’s license required.
