Enhanced Adult Mental Health Services (EAMHS) provides services to individuals with an SPMI diagnosis in an outpatient setting and in the community as needed. We work from a team based model to support our clients with a variety of services to help build skills for stability and independence.
The Community Case Manager assists with the implementation of treatment plans, provision of individual and group skill straining, case management, and coordination of multiple service providers forclient caseload with other care team members. Provides services primarily inthe community, but also onsite. Keeps accurate and timely records of activities.
Location: This position is based out of Life Works NW's Rockwood site.
Pay/Benefits: $25.39-$29.05 per hour, plus benefits
Salary Placement Notice:
The posted salary range reflects the compensation potential for this role. Most candidates will be placed between the entry level and midpoint of the range, depending on their experience and qualifications. However, more seasoned professionals with extensive exact or similar experience may be considered for placement toward the higher end of the range. Final salary offers are made in alignment with internal compensation processes in accordance with Oregon Pay Equity laws.
- Responsible forclient orientation including advising clients and significant other(s) ofthe services offered through the program and recommend services.
- Provides inputon clinical assessments and the development of treatment plans.
- Provides case-coordination by maintaining contact with other agencies, referral source(s) and staff regarding client and significant other(s) involvement in treatment services.
- Conducts individual and group skills trainings sessions based on current evidenced based practices e.g. collaborative problem solving, cognitive behavioral interventions, and solution focused therapy.
- Teaches socially acceptable behavior, employing techniques such as behavior modification and positive reinforcement. Encourages individuals and family members to develop and use skills and strategies for confrontingtheirproblems in a constructive manner.
- Observes and influences clients' behavior, communication and interaction. Detects unusual behavior and reports observations to professional staff.
- Organizes,supervises, and encourages clients to participate in social, educational,and recreational activities. Provides assistance and support to clients with the job search process.
- Assists theclient to the extent necessary, based on client's level of functioning, toaccess needed services and resources. Advocates on client's behalf.
May attend the client's psychiatric medical appointments, if clinically indicated. - Reviews and evaluates clients' progress in relation to measurable goals described in treatment plans.
- Provides clients or family members with information about mental health and addictions issues and about available services and programs, making appropriate referrals when necessary.
