We are seeking a dedicated Case Manager to support individuals living with severe and persistent mental illness
. In this role, you will help individuals build independence, maintain stability, and successfully integrate into their communities while improving overall quality of life.
Services are community-based and person-centered
, meaning support is provided at times and locations that work best for the individuals we serve. Our team focuses on fostering an environment of acceptance, empathy, hope, and recovery
.
- Provide community-based case management services to individuals with severe and persistent mental illness
- Support individuals in achieving independence, stability, and community integration
- Coordinate person-centered services to connect individuals with medical, social, educational, and community resources
- Assist individuals in identifying and developing natural supports within their community
- Advocate for consumer choice and individualized care
- Collaborate with clinical teams and community partners to ensure comprehensive support and care coordination
Education
- Bachelor’s degree in a Human Services field required (Social Work, Psychology, Counseling, or related field)
- Minimum of 1 year of experience in community mental health or a related human services field preferred
