$25-$50/hour

, the Licensed Social Worker (LI) supports residents and families through compassionate counseling, care coordination, and advocacy to ensure dignity, comfort, and quality of life.

Responsibilities for Licensed Social Worker (LI):
  • Evaluate patients (existing, new, and prospective) based on their psychosocial needs, strengths, and limitations.
  • Address patient concerns and goals while maintaining consistent communication with patients, families, and case managers.
  • Build and maintain strong, supportive relationships with patients and their families to promote trust and engagement in care.
  • Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments, to support comprehensive care planning.
  • Develop, implement, and update individualized care plans that reflect resident needs, goals, and progress.
  • Advocate for residents’ rights, emotional well-being, and access to appropriate services and resources.
  • Support discharge planning and transitions of care in coordination with the facility and external partners.
  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
  • Ensure compliance with all applicable laws, regulations, and standards governing social services in post-acute care settings.
Qualifications for Licensed Social Worker (LI):
  • College degree in Social Work or related field preferred and LI.
  • Must have prior long-term care experience.
  • Ability to make independent decisions when circumstances require sound clinical and ethical judgment.
  • Strong knowledge of social work practices, procedures, and applicable laws, regulations, and guidelines governing post-acute care facilities.
  • Ability to implement and interpret departmental programs, goals, objectives, policies, and procedures.
  • Proficient in social services responsibilities, patient rights, safety, and sanitation standards.
  • Demonstrates integrity and professionalism by adhering to the Marquis Code of Conduct and completing all required compliance training.
