Be Part of Our Growth! Join Our Team as an Independent Living Case Manager - Two Positions Available!

Our program is growing, and we're excited to add two Independent Living Case Managers to our team! This is an opportunity to make a meaningful difference in the lives of youth in foster care as they prepare for and transition into independent adulthood.

As an Independent Living Case Manager
, you'll provide direct support to youth in foster care, helping them successfully transition into independent adulthood. You'll develop individualized plans, coordinate with community partners, and empower young people by connecting them with the resources they need to thrive.

Ideal candidates demonstrate empathy, strong communication and listening skills, and the ability to maintain appropriate boundaries while delivering direct support.

What You'll Do:
  • Travel within Clearfield County (travel required; mileage/time reimbursed per policy)
  • Work 37.5 hours per week, 52 weeks per year
  • External Coordination: Build relationships with community partners, attend required meetings, and connect youth with resources like education, job training, and tutoring.
  • Direct Service:
    Set and monitor individual goals, complete biannual assessments, lead one-on-one and group sessions, and engage in interagency support.
  • Accountability:
    Submit timely documentation, meet billable hour requirements, and adhere to agency standards.
  • Internal

    Collaboration:

    Manage your schedule, coordinate with team members, and contribute to planning with the Therapeutic Recreation team.
  • Professional Development: Participate in training and workshops to build relevant skills.
What We're Looking For:
  • High school diploma;
  • 2- or 4-year degree in therapeutic recreation or related field
  • Experience working with youth
  • Valid driver's license
  • Reliable transportation for travel within the community.
  • Ability to work flexible hours, including some evenings to meet client needs.
  • Must be willing to obtain additional training when required for job duties.
  • Strong oral and written communication skills.
  • Excellent organizational and multitasking abilities
  • Ability to network/collaborate with internal and external team members.
  • Knowledge of treatment plans and experience working with adolescents
  • Ability to work with adults and adolescents.
  • Must obtain the following clearances prior to hire
    : PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate
