Job Summary

The Community Health Worker Supervisor will oversee the daily management of the Community Health Worker (CHW) workforce at CHW Institute at Montefiore Einstein. The position will guide, support and evaluate collective and individual CHW performance, including hiring, training, monitoring and coaching. The CHW supervisor will work closely with CHW Institute staff and senior management team, to effectively develop, maintain, and grow the CHW Institute.

This position will be engaged in all aspects, based on skill level, of implementation and ongoing management of direct service staff (CHWs). This position requires travel to multiple clinical sites for in person management and coordination.

Essential Functions
  • Oversee recruitment, orientation and training of CHW staff Interview candidates.
  • Provide program orientation (i.e. day-to-day workflow, program implementation and goals, review policies and procedures etc.)
  • Identify areas for improvement for CHW staff and provide coaching/training as necessary.
  • Identify external trainers when applicable
  • Ensure day to day operations of the CHW staff are aligned with CHW Center goals, protocols and policies and procedures.
  • Complete direct observations and provide one-to-one and group feedback
  • Present goals, progress towards goals (successes and areas of improvement) to CHW staff. Assign, manage and monitor caseloads or CHW staff.
  • Monitor incoming referrals
  • Assign referrals to CHW team
  • Monitor progress (i.e. primary contact, follow ups, service delivery and uptake)
  • Provide productivity and quality feedback to assigned CHWs
  • Coordinate and participate in case reviews, trainings and implementation planning.
  • Facilitate case reviews to improve the quality of CHW services (At minimum-monthly)
  • Develop, operationalize and optimize strategies to address social support services (i.e. food insecurity, housing, employment health system navigation etc.)
  • Assist in the development of social service navigation workflows
  • Lead quality improvement projects
  • Ensure timely and accurate CHW staff documentation and reporting; and Monitor program outcomes and identify areas for program improvement.
  • Complete chart reviews to ensure timely, accurate and comprehensive documentation is completed
  • Conduct regular meetings with CHW Team and key stakeholders to provide bi-directional feedback, share best practices and plan for navigation of challenges and/or gaps in services.
  • Facilitate monthly CHW team meetings
  • Attend Provider Champion and stakeholder meetings, provide updates on implementation successes and areas of improvement. Effectively communicate implementation successes and challenges with the Program Manager to allow for collaborative solution planning.
  • Provide quantitative and qualitative reports to Program Manager
  • Meet regularly (weekly/bi-weekly) with Program Manager
  • Provide performance feedback and conduct annual performance reviews for CHW staff.
  • Complete direct observations and provide one-to-one and group feedback
  • Complete annual performance reviews
Qualifications
  • Associates Degree Required
    - Bachelors Preferred
  • 2-3 year of supervisory experience or a minimum of 3 years of direct service as a Community Health Worker or in case management, care coordination, social service or related field Required.
  • Proficiency with Microsoft Word, PowerPoint, and Excel is required
  • Experience in the public health field; familiarity with community health workers preferred.
  • Knowledge of the local community, culture and resources, is favorable.
  • Bilingual in Spanish or French preferred.
