Position Summary

We are seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. Under the direction ofa licensed Physical Therapist, the PTA will provide skilled therapy services designed to help residents improve mobility, strength, function, and independence.

Key Responsibilities
  • Provide physical therapy treatments as directed by the supervising Physical Therapist
  • Implement individualized treatment plans based on physician orders and therapy goals
  • Monitor resident progress and communicate changes to the Physical Therapist
  • Document treatment sessions, resident responses, and progress notes accurately and timely
  • Assist with discharge planning and caregiver education
  • Collaborate with nursing staff and interdisciplinary team members to achieve positive resident outcomes
  • Participate in care conferences and team meetings as needed
  • Maintain a clean, safe, and organized therapy environment
  • Ensure compliance with all state, federal, and facility regulations
  • Support residents in achieving their highest level of function and independence
Qualifications
  • Education & Experience Graduate of an accredited Physical Therapist Assistant program
  • Experience in skilled nursing, rehabilitation, or long-term care preferred
  • Experience with Point Click Care  (PCC) and Casamba preferred
  • Strong documentation, communication, and organizational skills
Licenses & Certifications
  • Current South Carolina Physical Therapist Assistant (PTA) license in good standing
  • Must maintain all required continuing education requirements
  • Must remain in good standing with the State Board
