Clinical Coordinator - Medical Assistant or LPN

Underthegeneraldirectionofthe Medical Director ,the Clinical Coordinatorsupportsthewellbeingofall Family  Health Centerpatientsbyensuring timely communication,action,andfollow upona widevarietyof patient related concerns.

The Clinical Coordinatorfunctionsasaliaisonbetweenpatientsandmembersofthecare  teamthroughcommunication(viaphone,fax,electronichealthrecord,in-person,or otherwise),carrying out provider instructions(i.e.,communicating lab results,refilling prescriptions,etc.),andmaintainingmeticulous documentation.

• Collaborate with internal and external care team members topromote patient safety and to optimize patient health outcomes.
• Prioritizetasksbasedonpatients'immediateneedsandorganizationalpriorities.
• Answer patient phone calls, gather patient-reported information, and orchestrate timely follow up on awidevarietyofpatientneeds.
• Support the patient care team by maintaining accurate patient records, sending and receiving faxes, and followinguponpatient-centeredtaskssuchasmedicationrefillrequests.
• Notifypatientsoflabresults(bothnormalandabnormalresults,asindicatedandunder,thedirectionof a provider).
• Document clinical actionsin the electronic health record (EHR) and organize documentation so that p tient recordsareaccurate,complete,and accessible.
• Utilize the EHR to schedule appointments, communicate with members of the care team,and provide patient education and/or recommendations as outlined bya provider.
• Supportandadheretoadministrativeandclinicalpoliciesandprocedures.
• Functionasapatientadvocate.
• Participateinroutinecompetencytraining,in-service education,anddepartmentalmeetings.
