What You’ll Be Doing (High Level)
  • Providing skilled PT care in patients’ homes
  • Completing admissions and routine visits
  • Developing and carrying out individualized care plans
  • Collaborating with nurses, therapists, and providers
  • Participating in interdisciplinary team meetings (The formal job description comes later—this is about the experience.)

What You’ll Need
  • Oregon PT license (or ability to obtain before start date)
  • 1–2 years PT experience (home health preferred, not required)
  • Current CPR certification
  • New PT graduates encouraged to apply
