As the Social Services Director
, you will play an important role in supporting the emotional, social, and overall well-being of our residents.

Responsibilities include:

• Oversee and direct social services operations within the facility

• Conduct psychosocial assessments and maintain required documentation

• Participate in care planning and provide appropriate input for MDS assessments

• Advocate for residents' emotional, social, and mental well-being

• Collaborate with the interdisciplinary team during care conferences and care planning

• Assist residents and families with discharge planning, transfers, and accessing community resources

• Build positive relationships with residents, families, and community partners

• Support smooth transitions of care and discharge planning

• Maintain compliance with applicable state and federal regulations

• Help support resident engagement and quality-of-life initiatives

• Provide guidance and leadership within the social services department

• Bachelor's degree in social work or a related field preferred

• Current Colorado social work licensure or eligibility preferred

• Previous experience in skilled nursing, rehabilitation, or long-term care strongly preferred

• Knowledge of MDS, care planning, and regulatory requirements preferred

• Strong communication, organizational, and leadership skills

• Compassionate and committed to resident advocacy and person-centered care

• Ability to work collaboratively with residents, families, and an interdisciplinary healthcare team
