Salary:
Steps 2-4: $5,102.22-$5,600.20;
Steps 5-8: $5,868.01-$6,754.50

Position Summary:
This position will have responsibility for responding to behavioral health-related 911 calls and providing problem identification, crisis interventions, referrals, and linkage to resources for individuals in Chelan and Douglas County. The Behavioral Health Unit Outreach Case Manager will be primarily responsible for face-to-face interventions, temporary transitional case management, assessments, and specific law enforcement second-responder duties.
The Outreach Case Manager provides outreach services to individuals experiencing acute emotional crises, to divert the individual from incarceration or hospitalization into the least restrictive setting possible, and conducts assessments across systems to determine eligibility for diversion into safe and appropriate treatment options. When appropriate, coordinates requests for evaluation under the Involuntary Treatment Act and appropriately utilizes crisis intervention techniques for individual and community stabilization.
- Works collaboratively to assist and support law enforcement in Chelan and Douglas on calls involving individuals in a mental health/substance use-related crisis.
- Functions as the primary point of contact for law enforcement.
- Participates in regular ride-alongs in all partner police departments.
- Works closely with community members who are in crisis, identifying signs of mental health conditions, emotional dysregulation, and substance use.
- Performs risk assessments, crisis interventions, and employs de-escalation techniques.
- Provides community-based care through effective and efficient in-person responses, telephone support, and referrals.
- Provides transitional case management and supports planning and utilization of resources to stabilize individuals in Crisis Stabilization or Shelters, reduce the length of hospital admission and incarceration, and decrease the frequency of 911 utilization.
- Acts as Liaison between Jail Administrators and Mental Health Professionals, Probation, Department of Corrections, and City/County Attorneys
- Completes Jail Requests for Services and makes appropriate referrals for jail intake and transition services.
- Acts as liaison to behavioral health and other community social and support services for the continued care of individuals.
- Coordinates with Crisis Services/DCRs, treatment facilities, EMS, and Emergency Room staff
- Supports the use of engagement and creative problem-solving with individuals and their natural supports to facilitate the least restrictive placement that meets the consumer’s and community’s needs
- Develops and maintains a thorough knowledge of community resources relevant to crises being managed.
