The Metro Health System is redefining health care by going beyond medical treatment to improve the foundations of community health and well-being: affordable housing, a cleaner environment, economic opportunity and access to fresh food, convenient transportation, legal help and other services. The system strives to become as good at preventing disease as it is at treating it. Founded in 1837, Cuyahoga County's safety-net health system operates four hospitals, four emergency departments and more than 20 health centers.

Summary:

Helps patients with substance use disorders (SUD) and their families navigate successfully through the health care system by removing barriers to care, including providing access to resources, transportation, and support and enhancing communication with the health care providers. Treatment includes, but is not limited to, regularly performing SUD assessments, linkages to ongoing treatment, linkages to community resources, educating patients, communicating with clinical and administrative staff, monitoring and reporting outcomes, and participation in other meetings and activities.

Ensures follow-up regularly occurs with the identified patient population for up to 6 weeks or longer as necessary following hospital discharge. Upholds the mission, vision, values, and customer service standards of The Metro Health System.
