In order to be considered for this job, you must have an active and unrestricted Licensed Clinical Social Worker license in the state of Kentucky.

This position is a per diem position and can be offered either as a 1099 independent contractor role or a W2 employee role.

Pay is $60-$70 per visit depending on being a 1099 contractor or W2 employee.

The Licensed Clinical Social Worker (LCSW) will provide individualized counseling, assessment, and therapeutic services to patients in the comfort of their own homes. The ideal candidate will have experience in healthcare, be able to work independently, and have a passion for improving the mental and emotional well-being of patients with chronic health conditions, elderly populations, and those facing significant life transitions.

• Conduct in-depth psychosocial assessments of patients and their families in homecare settings to identify emotional, social, and psychological needs.
• Provide individual and family counseling services, including crisis intervention and supportive therapy, to help patients cope with illness, aging, or other health-related challenges.
• Collaborate with a multidisciplinary healthcare team, including physicians, nurses, and therapists, to develop and implement personalized care plans.
• Educate patients and their families about coping mechanisms, available community resources, and support networks to manage chronic illnesses, disabilities, and emotional challenges.
