Physical Therapy Assistant - Home Health

Role Overview:

This position involves providing physical therapy assistance to patients in their homes, focusing on treatment, exercise instruction, and patient education to enhance well-being.

• Following state laws and supervising PT guidelines to deliver safe, qualified care.
• Instructing patients in exercises, gait, ADL, transfer, safety, and prosthetic training.
• Monitoring, recording, and reporting patient responses and condition changes.
• Collaborating with healthcare team members and educating patients, families, and caregivers.
• Adapting to diverse home environments to ensure effective care.

• Licensed or certified to assist in physical therapy within the state.
• Valid driver's license, dependable transportation, and availability for weekend work.
• Preferred: CPR certification and physical ability for mobility support tasks.
