Logan Health has an opportunity for an Ambulatory Pharmacist to join their team in our Oncology Clinic!

As a key member of our pharmacy team, this position will play a vital role in supporting patients prescribed oral oncology treatments. This position ensures continuity of care by monitoring lab results, managing adverse effects, performing medication reconciliation, and providing counseling to patients. You'll also collaborate closely with our specialty pharmacy team to address medication access and availability. Additionally, you’ll serve as a trusted clinical resource to providers and nursing staff.

** This position will be working out of our Logan Health Hematology/Oncology Clinic located on the Logan Health campus in beautiful Kalispell, MT. *** Experienced in ambulatory care pharmacy settings
* Knowledgeable in oncology, especially managing specialty medications
* Ideally completed a PGY2 residency in Ambulatory Care, Oncology, or related area
* BCOP certified or actively pursuing Oncology Pharmacy Specialty Certification

* Qualifications:

*** PharmD from a College of Pharmacy accredited by American Association of Colleges of Pharmacy, or equivalent clinical experience required.
* Current Montana State Board of Pharmacy, Registered Pharmacist License required.
* Oncology Pharmacy Specialty Certification (BCOP) preferred
* Completion of PGY2 in Ambulatory Care, Oncology or related area preferred
* Accurate data entry skills, analytical and advanced math skills required.
* RQI required and as applicable to assigned area(s):  + First Responder required for assignment:
Clinic, Retail, Behavioral Health, IS and Administration Pharmacists.  + ACLS required for all other assigned area(s).

Specific Duties :
*** Evaluates and verifies orders for therapeutic appropriateness, proper dosing, optimal dosage form, drug interactions, allergic reactions, route and rate of administration, and therapeutic redundancy.
* Provides patient-specific pharmacotherapy recommendations and drug information to the appropriate stakeholders as applicable to assigned area(s).
* Performs daily review of pharmacy reports including pharmacokinetic monitoring as applicable to assigned area(s).
* Documents clinical interventions appropriately in electronic medical record.
