Social Worker, Mental Health

• Perform psychosocial assessments
• Develop and implement care plans with the appropriate care team
• Assess crisis situations and provide clinical counseling, diagnosis, brief therapeutic interventions, resources, or referrals
• Provide individual and family treatment as indicated
• Evaluate patients for transition planning, mental health, substance use, and goals of care
• Coordinate care and address social determinants of health and community resources
• Use motivational interviewing and therapeutic techniques, including trauma-informed care and substance use disorder support
• Provide brief individual, group, and family therapy
• Identify mental, emotional, and behavioral disorders and connect patients to services
• Train staff, educate patients, and advocate for rights and care access
• Collaborate with healthcare teams, insurers, community providers, patients, and support persons
• Lead improvement initiatives, track key metrics, and ensure policy adherence
• Facilitate clinical goals-of-care discussions with patients, families, and teams
• Participate in required Microsoft Teams video interviews and potential onsite interviews and meetings

• Master of Social Work (MSW) from an accredited institution; degree verification required
• Current state licensure, as applicable, obtained prior to or upon completion of required supervision hours (CSW/LCSW)
• Basic computer proficiency, including word processing and spreadsheet software
• Strong written and verbal communication skills
• Demonstrated critical thinking skills
• Case Management Certification preferred
• Experience in clinical care management, social work, or working with third-party payers preferred
• Understanding of care management principles and practices preferred
• Understanding of health insurance products and related processes preferred
• Understanding of coding, episode of care, and length of stay guidelines preferred
  
  For duties requiring home or community visits: current BLS certification, driver's license, auto insurance, acceptable driving record, and reliable transportation

Demonstrates expertise in psychosocial assessments, care plan development, and crisis intervention while effectively collaborating with healthcare teams and advocating for patient rights. Proficient in motivational interviewing and trauma-informed care, with a strong focus on mental health and substance use support.
