** Pay Range:** $20.00 - $25.00 per hour## Position Summary Hemet Hills Post Acute is seeking a compassionate and organized
** Social Services Assistant
** to join our interdisciplinary team. The Social Services Assistant supports residents and their families by helping coordinate services, facilitating communication, assisting with discharge planning, and contributing to a positive resident experience. This role is ideal for someone who is passionate about serving seniors and individuals in a skilled nursing and rehabilitation setting.##
Essential Duties and Responsibilities
* Assist the Social Services Director with resident assessments, care planning, and discharge planning activities.
* Support residents and families by providing information regarding available resources and community services.
* Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
* Coordinate appointments, transportation, and referrals as needed.
* Help address resident concerns and support resident rights and quality of life initiatives.
* Participate in interdisciplinary team meetings and care conferences.
* Assist with admissions, transfers, and discharge processes.
* Promote a positive and supportive environment for residents, families, and staff.
* Maintain confidentiality and comply with HIPAA regulations.## Qualifications
* High school diploma or equivalent required.
* Associate's or Bachelor's degree in Social Work, Human Services, Psychology, or a related field preferred.
* Previous experience in a healthcare, skilled nursing, rehabilitation, or social services setting preferred.
* Strong interpersonal, communication, and organizational skills.
* Ability to work effectively with residents, families, and healthcare professionals.
* Proficient with basic computer applications and electronic health records preferred.
* Ability to maintain professionalism, empathy, and confidentiality in all interactions.## Benefits
