Palo Verde Behavioral Health Hospital isseekinga compassionate, motivated, and Master's-level Social Worker to join our supportive
Inpatient team. - As a vital member of our treatment team, you'll help guide patients through their recovery journey by providing support, coordination, advocacy, and education. Every day offers an opportunity to make a meaningful difference in the lives of those we serve.
- Be the Connection: Collaborate with referral sources, healthcare providers, and community partners to ensure seamless patient care. Participate in daily interdisciplinary team meetings, coordinate follow-up appointments, facilitate Adult Recovery Team meetings, and support urgent patient needs.
- Build Strong Relationships: Maintain regular communication with patients, families, and treatment professionals, ensuring everyone involved in the recovery process stays informed and supported.
- Champion Patient Engagement: Encourage patients to actively participate in their treatment journey, including group therapy, treatment planning, and discharge planning activities.
- Empower Recovery: Connect patients with valuable community resources and support services that promote long-term wellness and successful recovery after discharge.
- Ensure Compliance and Quality Care: Adhere to program guidelines, treatment protocols, and HIPAA requirements while supporting the highest standards of patient confidentiality and care.
- Advocate for Patients: Complete excellent documentation to support patients inan appropriate lengthof stay and acceptance to referral Level of Care.
- In this role, you'll be more than a clinician or care coordinator—you'll be a trusted guide, advocate, and source of encouragement for individuals working toward recovery. Your contributions will help patients build the skills, support systems, and confidence they need to achieve lasting success.
Education: Masters or Doctoral degree in Social Work, Psychology, Professional Counselingor related degree.
Experience: Experience coordinating care, familiarity with community resources, and ability to be self-organizing. Experience in the field of Behavioral Health for at least 2 years is preferred, does not have to be obtained post-Master's
Licensure/Certification: No license is required
