Social Worker
Job Summary:

At Saunders Nursing and Rehabilitation Center, we are committed to providing exceptional care and services to older adults in our community. We are seeking a compassionate and dedicated Social Worker to join our team and play a vital role in supporting the physical, emotional, and social well-being of our residents.

About the Social Worker Role:

As a Social Worker at Saunders Nursing and Rehabilitation Center, you will work closely with our interdisciplinary team to assess and address the social and emotional needs of our residents. Your expertise will help identify and address challenges, develop and implement care plans, and provide support to residents and their families. You will also collaborate with other healthcare professionals to ensure a comprehensive approach to resident care.

Social Worker Responsibilities:
  • Conduct social assessments and develop individualized care plans to address the social, emotional, and spiritual needs of residents
  • Provide counseling, guidance, and support to residents and their families to address challenges and concerns
  • Collaborate with other healthcare professionals to develop and implement care plans
  • Identify and address social and emotional issues that impact resident well-being
  • Advocate for residents' rights and needs to ensure they receive high-quality care and services
  • Maintain accurate records and reports of resident interactions and care plans
  • Participate in facility-wide committees and initiatives to improve resident care and services
Social Worker Requirements:
  • Bachelor's degree in Social Work or related field
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities
  • Excellent communication, interpersonal, and problem-solving skills
  • Ability to work collaboratively with a diverse team of healthcare professionals
  • Demonstrate a commitment to providing exceptional care and services to older adults
