Job Purpose

The Health Liaison Team Lead supports the Clinical Development Officer in the coordination, leadership, and oversight of Health Liaison services focused on providing primary care screening and monitoring to improve client health outcomes and overall well-being. This role is responsible for supervising Health Liaison staff, fostering evidence-based and client-centered practices, ensuring compliance with organizational standards and policies, and promoting ongoing staff development to support the delivery of high-quality, client-centered care.

Qualifications
  • High School diploma or equivalent.
  • Psychiatric/Behavioral Health experience preferred.
  • Healthcare technology experience preferred.
  • Ability to perform basic medical assistant job duties.
Core

Job Duties
  • Manage and oversee a caseload of clients with a focus on primary care screening, monitoring, and overall wellness support.
  • Ensure completion of all initial and ongoing screenings in accordance with the Primary Care Screening and Monitoring Policy.
  • Conduct health screenings as needed to identify potential health risks and support early intervention efforts.
  • Obtain and review screenings completed by clients' Primary Care Physicians (PCPs) and ensure documentation is uploaded into the Electronic Medical Record (EMR) system.
  • Provide ongoing health education and wellness support tailored to clients' identified health conditions and needs.
  • Coordinate, initiate, and monitor referrals to primary care and specialty healthcare services.
  • Assist clients with scheduling appointments for primary care and specialty services as needed.
  • Monitor attendance at external healthcare appointments to support continuity of care and follow-through with treatment recommendations.
  • Coordinate care with PCPs and specialty providers, with appropriate client consent, to promote integrated and collaborative healthcare services.
  • Encourage and support clients in following prescribed treatment plans and medical recommendations.
  • Collaborate with FCC providers and interdisciplinary teams to develop comprehensive, client-centered care plans.
  • Maintain accurate and timely documentation of health education, referrals, appointment assistance, and coordination with healthcare providers.
  • Support the Clinical Development Officer (CDO) in the oversight and daily operations of the Health Liaison program.
  • Provide guidance and assistance to Health Liaisons in carrying out daily responsibilities and client care activities.
  • Complete 4 Chart Audits monthly and inform CDO of any issues.
  • Perform Supervision with the Health Liaisons and report any issues to the CDO.
  • Assist in leading strategic efforts to enhance the agency's primary care and monitoring services.
  • Adherence to all applicable evidence-based practice models.
  • Orally communicate information effectively and accurately.
  • Assure program operates within the constraints of the agency's certification (CARF, DMH) standards and strategic plan related to clinical care.
  • Abide by program and agency policies and procedures.
  • Other job duties and special projects as assigned.
