We are seeking caring and dedicated Medical Social Workers to join our team and make a meaningful difference in the lives of patients and their families.
As a
** Medical Social Worker at Avo Care**, you will provide emotional support, counseling, and advocacy to patients and their loved ones as they navigate the challenges associated with serious illness. You will assess psychosocial needs, assist with advance care planning, connect patients and families with community resources, and collaborate closely with physicians, nurses, and the interdisciplinary care team to ensure holistic, patient-centered care.

* Job Title:
Social Worker (Palliative Care)
** Purpose of the Position:
Provides support to patients who would benefit from psychosocial and emotional intervention. Offers necessary community resources coordination to patients and families who need assistance with navigating the complex healthcare system. Responsibilities:
● Coordinate and provide home visits to patients
● Adhere to HIPPA at all times
● Documents psycho-social assessment, patient status, and outcomes after each visit
● Ensure regulatory compliance with documentation requirements
● Assist the palliative team and/or directly facilitates patient-family meetings for goal setting and care planning
● Provides education and emotional support to patients and families
● Provides community resources and referrals as needed to patients/caregivers
● Support patient and family through referral to hospice
● Participates in discharge planning
● Participates quality improvement initiatives
● Attends and actively participates in IDT meetings, staff meetings, trainings
● Performs other duties as assigned

* Job Requirements:
** 1. Ability to travel per job requirements
2. Thorough knowledge of community resources or desire/ability to obtain resources
3. Knowledge of the psychosocial process in chronically and seriously ill patients and their families
4. Capacity to relate and work well with others as a member of an interdisciplinary team; this must include the ability to give and accept objective criticism constructively.

* Job Qualifications:
*
* ● MSW or BSW degree in social work from a school of social work accredited by the Council on Social Work Education
● Minimum of 2 years experience as social worker in healthcare
● Preferred experience as a social worker in community based care ie. Home Health, Hospice,Palliative Care.
