It happens in a client’s home. In the community. During the everyday moments when someone is learning how to manage symptoms, build confidence, solve problems, reconnect with others, and live more independently.
At Advocate Support Services, Inc., our Case Workers provide community-based Psychiatric Rehabilitation Program (PRP) services to adults living with serious mental illness.
This is a relationship-driven position for someone who can connect with people, work independently in the community, stay organized, and consistently follow through.

. Our Case Workers build consistent, trusted relationships with clients and help them develop the practical skills and supports necessary to succeed in everyday life.
As a Case Worker, you will manage an assigned caseload of adults receiving PRP services and provide individualized rehabilitation services in homes and community settings.
Your responsibilities will include:
- Meet with clients regularly in their homes and communities to provide skill-building and rehabilitation services
- Help clients develop practical skills related to:
- Activities of daily living
- Coping skills and emotional regulation
- Medication and health-management routines
- Budgeting and financial organization
- Transportation and community navigation
- Social skills and community integration
- Appointment follow-through
- Accessing community resources
- Independent living and personal goals
- Develop meaningful, professional relationships that promote engagement and progress
- Maintain consistent contact with assigned clients and proactively address missed or difficult-to-schedule visits
- Plan and organize your weekly schedule to meet client needs and program expectations
- Coordinate with therapists, psychiatrists, healthcare providers, family members, and community partners when appropriate
- Work collaboratively with Rehabilitation Specialists and program leadership
- Support implementation of each client’s Individual Rehabilitation Plan (IRP)
- Complete accurate, individualized documentation in the electronic health record
- Complete documentation promptly and in accordance with program and regulatory requirements
- Participate in supervision, team meetings, training, and quality-improvement activities
Successful Case Workers at Advocate:
- Build genuine relationships with clients
- Consistently complete scheduled client encounters
- Manage approximately 20 active clients in a full-time caseload
- Organize their schedules effectively and maintain consistent client engagement throughout the month
