27th August, 2026 About the RoleWe are seeking a compassionate, reliable, and experienced Community Support Worker to join our team on a permanent part-time basis. This role involves providing person-centred support to individuals with complex care needs, neurological conditions, enabling them to achieve their goals and maintain independence, dignity, and quality of life.
The successful candidate will have experience supporting participants with high care needs and neurological conditions and be committed to delivering exceptional care in accordance with individual support plans and organisational policies.
This is a permanent part-time position offering 30 hours per fortnight
. You will be required to travel throughout Albany and surrounding suburbs.
You need to be available from Monday to Friday between 6:00am - 6:00pm or 7:30am

- Deliver high-quality, person-centred support to participants in their home and community environments.
- Assist with personal care, including showering, grooming, dressing, toileting, and continence care.
- Support participants with mobility, transfers, and use of assistive equipment.
- Provide support for individuals with neurological conditions such as acquired brain injury, multiple sclerosis, Parkinson's disease, motor neurone disease, and other complex health needs.
- Administer or assist with medication in accordance with organisational policies and training.
- Support participants to attend appointments, access community activities, and develop daily living skills.
- Monitor participant wellbeing and promptly report changes in health or behaviour.
- Complete accurate documentation, progress notes, and incident reports.
- Maintain professional relationships with participants, families, health professionals, and team members.
- Adhere to NDIS Practice Standards, workplace health and safety requirements, and organisational policies.
