Direct Care & Transport Specialist

Main Role:
Provide direct care and supervision for members, help them follow their daily schedules, and safely transport them to activities, appointments, and home.

• Provide daily care and supervision to members.
• Help members follow rules, schedules, and daily activities.
• Assist with self-administration of medication.
• Transport members safely to activities, appointments, and outings.
• Ensure member safety in the facility and in vehicles.
• Help members with daily living tasks, including bathroom assistance when needed.
• Support exercise and wellness activities.
• Help with admissions and discharges of members.
• Write daily progress notes and complete paperwork (attendance, mileage logs, etc.).
• Report incidents to supervisors.
• Protect member rights and confidentiality.
• Help with food service and cleaning of the facility.
• Follow safety and disaster plans if needed.
• Complete required training (CPR, First Aid, job training).

• Must have a valid Arizona driver’s license.
• No cell phone use while working.
• No sleeping on shift.
• Must wear full uniform during work hours.

• Able to lift up to 50 lbs.
• Able to drive a vehicle.
• Able to stand for long periods and stay active.
• Able to multitask and stay organized.
• Participate in indoor and outdoor activities with members.
