Responsibilities- Evaluate residents and patients and develop individualized treatment plans
- Provide skilled physical therapy services in accordance with physician orders and established plans of care
- Educate patients, families, and caregivers regarding treatment goals and therapeutic interventions
- Monitor patient progress and modify treatment plans as appropriate
- Complete timely and accurate documentation in accordance with company policies and state and federal regulations
- Communicate patient status and progress with physicians, nursing staff, and interdisciplinary team members
- Assist with scheduling and accurately complete daily treatment charges
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license in Pennsylvania (or eligibility to obtain)
- Strong communication and interpersonal skills
- Passion for providing exceptional patient care
