- Evaluate patients and develop individualized treatment plans.
- Provide skilled physical therapy services in accordance with physician orders and care plans.
- Supervise Physical Therapist Assistants (PTAs) in compliance with state practice regulations.
- Collaborate with interdisciplinary team members to coordinate and optimize patient care.
- Participate in care conferences, rehabilitation meetings, and utilization reviews as needed.
- Maintain timely and accurate documentation, including evaluations, treatment notes, progress reports, and discharge summaries.
- Educate patients, families, and caregivers on treatment goals, mobility strategies, safety techniques, and home exercise programs.
- Assist with discharge planning and recommendations for continued care.
- Recommend and coordinate appropriate durable medical equipment (DME).
- Ensure a safe treatment environment by monitoring and reporting equipment maintenance needs.
- Current Physical Therapist (PT) license in good standing.
- Strong communication, organization, and interpersonal skills.
- Passion for delivering exceptional patient‑centered care.
- Ability to work effectively within a multidisciplinary team.
