Position: DIABETIC EDUCATOR
Location: Northern

JOB REQUIREMENTS

  • Education
    • Registered Nurse (RN) or Registered Dietitian(RD).
    • If an RD, must have completed an internship program approved by the Academy of Nutrition and Dietetics or meet the experience requirements to become a Registered Dietitian. If registry eligible, the employee must pass the registration examination within the first six months of employment. Previous experience in community education or health education is preferred.
    • If RN, a Bac c a l a u r e t e degree in Nursing is required, Master’s degree preferred. Previous experience in community education desired.
  • Experience
    • Two years of experience in a health care and/or health promotion setting is required. Must demonstrate the ability to teach in a variety of settings and to diverse audiences. Experience in development of written course materials, flyers, advertisements, etc. as needed.
  • Physical
    • Normal hospital environment. Close eye work. Hearing of normal and soft tones. Frequent exposure to biological hazards. Distinguish smells. Distinguish temperatures by touch. Operates AV equipment. Continuous walking, frequent standing. Lifting, carrying up to 40lbs. Pushing/pulling up to 150lbs.

JOB SUMMARY
  • The Diabetes Educator provides comprehensive diabetes education, nutrition counseling, and health and wellness education to patients, families, and community members in outpatient, inpatient, and community settings. This position assesses educational and nutritional needs; develops, implements, evaluates, and documents individualized education and nutrition therapy plans; and provides evidence-based education to support diabetes management, disease prevention, and healthy lifestyle behaviors.

    The Diabetes Educator demonstrates effective adult teaching and learning skills and serves as a resource to patients, families, staff, and the community. Excellent communication and public speaking skills are essential. The position promotes positive customer relations, collaborates with the healthcare team, participates in performance improvement initiatives, assists with staff education and training, and may direct or assign work to staff as appropriate.

    The Diabetes Educator maintains professional competency through continuing education and exercises independent judgment in the performance of assigned responsibilities. The position serves as a role model for health promotion, wellness, and professional practice.
