Our Pediatric Specialty Clinic is looking for a Diabetes Educator to join the team! This role will help coordinate care to ensure families and patients have the treatment, education and resources necessary to manage their diabetes. In addition, our staff along with our pediatric endocrinologists, go to several outreach clinics throughout the region, allowing for increased access to the specialized care that is essential to their health and wellness.

Our clinic is open Monday – Friday 8:00am – 5:00pm, no holidays or weekends. This position provides shared call coverage amongst team members on weekends and holidays.

Job Summary

Educates patients on self care involved in the diagnosis of diabetes; collects patient data to monitor health status and

proactively manage the patient's condition. Provides initial and ongoing assessment, education, goal setting and behavioral interventions to promote diabetes self management skills and patient empowerment. Ensure patient/family have understanding of self-care skills including healthy eating, medication management, physical activity, glucose monitoring/pattern management, problem solving, and reducing risks. Demonstrates knowledge of pathophysiology, social determents of health, and evidence-based standards. Supports quality improvement to effectively maintain program recognition for an effective Diabetes Self Management Education program.

Collaborates with multidisciplinary team, referring provider and other healthcare providers related to diabetes care.

Provides nutrition therapy and monitors nutritional care of clients/patients of all ages. Ability to recognize, evaluate, and provide for nutritional needs based on cultural, ethnic, and religious beliefs. Develops an appropriate age, individualized education and care plan based on assessment. Evaluates patient needs and suggests appropriate management strategies, including insulin management and diabetes technologies. Provides training and support to patients and families related to diabetes management technologies (continuous glucose monitors and insulin pumps)

as appropriate. CDCES will maintain certification on insulin pumps based upon location requirements. Maintains practice by staying abreast of current evidence-based practice. Follows facility protocols related to diabetes care management. May provide education or community support related to diabetes management or diabetes prevention. Responsible for working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients.

Qualifications

Minimum of a bachelor’s degree in dietetics or food and nutrition through an Accreditation Council for Education in Nutrition and Dietetics (ACEND) accredited program. For graduates after 2024, a Masters degree is required per ACEND.

Clinical nutrition experience through an approved Coordinated Graduate Program or post graduate internship is required. Three years of experience as a Registered Clinical Dietitian. Must have a thorough knowledge of normal nutrition, diabetes self-management skills and medical nutrition therapy.
