Position Summary

The Home Care Coordinator oversees and manages the delivery of home support and personal care services to clients in their homes. This role ensures that clients receive high-quality, compassionate, and compliant care in accordance with agency policies, state regulations, and individualized care plans. The Coordinator serves as the main liaison among clients, caregivers, service coordinators and families.

Key Responsibilities
  • Client Intake & Assessment

    • Conduct initial client assessments to determine care needs.

    • Develop and implement individualized care plans in collaboration with client and/or family members and service coordinators.

    • Reassess client needs regularly and update care plans as needed.

  • Care Coordination

    • Schedule and assign caregivers based on client needs, staff availability, and geographic location.

    • Monitor service delivery to ensure compliance with care plans, quality standards, and regulatory requirements.

    • Communicate regularly with clients and families to ensure satisfaction and address any concerns.

    • Coordinate with external providers (e.g., hospitals, clinics, social services) to facilitate transitions of care.

  • Staff Supervision & Support

    • Recruit, train, and supervise caregivers.

    • Provide ongoing coaching, performance evaluations, and support to field staff.

    • Ensure all caregivers maintain required certifications and training.

  • Administrative & Compliance

    • Maintain accurate and timely documentation in client records and agency systems.

    • Ensure compliance with HIPAA, state licensing, and agency policies.

    • Prepare reports on service utilization, client outcomes, and staffing.

    • Participate in quality assurance and improvement initiatives.

Qualifications
  • Education:

    • High School Diploma Level

  • Experience:

    • 2–4 years of experience in home care, case management, or healthcare coordination.

    • Supervisory or scheduling experience preferred.

  • Skills:

    • Strong organizational and multitasking abilities.

    • Excellent communication and interpersonal skills.

    • Knowledge of home care regulations, care planning, and documentation standards.

    • Proficiency with electronic health records (EHR) and scheduling software.
