SHIFT:

Fulltime;
Monday through Friday, 7:30am to 4:30pm on location during school session;
Monday through Friday 8:00am to 5:00pm in office summer and holidays

JOB PURPOSE:

Provides services to children/adolescents with identified needs as part of early intervention support toward developing the whole child; case management, skills training, and medication training to children/adolescents with a serious emotional disturbance; and provides services to the primary caregiver or LAR. Based on a school campus and integrated with school personnel and provides services in the school setting and/or community to increase accessibility and engagement to necessary services in rural communities.

Collaborates and partners with School Counselor. Social Emotional Behavior Support Specialist (SEBS), and Administrators at the school.

EDUCATION AND EXPERIENCE:

Required:

A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)

Preferred:
At least six months experience in providing mental health services to adults, children and adolescents.

DEGREE AND/OR CERTIFICATION

REQUIRED:

BA/BS in field specified above or licensed as a Registered Nurse in the State of Texas.

RESPONSIBILITIES:

2.Provides School Based Case Management services according to Center Policy and Contract requirements
3.Provides Crisis Intervention services according to HHSC Contract and Center Policy.
4.Provides case management, skills training, and medication training in accordance with Medicaid Rule, HHSC Contract, and UM Guidelines
5.Completes CANS assessment in accordance with Center policy and HHSC Contract.
6.Formulates a plan of care to identify the appropriate skill areas, frequency and duration of services to be provided in conjunction with the consumer/families and based upon the CANS.
7.Provides ninety-five percent (95%) of services in the consumers' natural environment (including at home, school, and community).
10.Submits all documentation including forms, services, disclosures, and telephone contacts etc within two (2) business days.
11.Tracks and manages caseload to avoid expiration of Consumer's required documents and forms to avoid lapse in services.
