Compensation Range: $18/hr - $19/hr commensurate with experience

Benefits:
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.

Overview:

As part of the support services team at The Lirio, the Case Aide provides day-to-day support to residents as they work toward long-term housing stability, wellness, and community integration. The Case Aide works under the direction of Case Managers and Program Leadership to assist residents in implementing service plans, developing independent living skills, accessing needed services, and maintaining safe and stable housing.

All services at The Lirio are provided through the lens of harm reduction, trauma-informed care, resident choice, recovery, and housing retention. To that end, the Case Aide will help residents remain engaged in supportive services, strengthen daily living skills, navigate appointments and benefits, and bridge the gap between residential support and formal clinical case management.

Responsibilities:

Primary:

  • Build rapport with residents and provide supportive, respectful, and professional engagement on a daily basis.
  • Assist residents with activities and goals identified in their service plans, including housing stability, health care engagement, behavioral health treatment, medication adherence support, benefits maintenance, daily living skills, community integration, and recovery-oriented goals.
  • Accompany residents to community-based appointments, including medical, behavioral health, HIV care, benefits, legal, and social service appointments, as needed.
  • Utilize motivational interviewing, harm reduction, trauma-informed, and person-centered approaches to encourage resident participation and progress.
  • Monitor and encourage resident participation in program activities, groups, wellness services, and community-building opportunities.
  • Document resident interactions and service notes accurately and timely in the Electronic Case Record system.
  • Communicate observed resident behaviors, concerns, needs, or accomplishments to the assigned Case Manager and participate in case discussions as requested.
  • Support new resident orientation and move-in processes, including assistance with understanding building expectations, available services, and community resources.
  • Participate in regular safety and wellness checks and collaborate with Residential Aides and other team members to maintain a safe, stable, and welcoming living environment.

Key Performance Indicators (KPIs):

  • Maintain timely, accurate documentation of resident contacts, service support, wellness checks, and appointment assistance in the Electronic Case Record system.
  • Support resident engagement in service plans, program activities, wellness services, and community-building opportunities.
  • Communicate resident needs, concerns, accomplishments, and changes in presentation to Case Managers and Program Leadership in a timely manner.

Other functions that may be needed with this position include:

  • Demonstrate excellent customer service with residents, staff, vendors, and all other relevant stakeholders.
