- Assist the Social Services department with supporting residents and their families.
- Help residents adjust to the skilled nursing environment and address social and emotional needs.
- Assist with completing and maintaining resident documentation and records.
- Communicate with residents, families, staff, and outside resources as needed.
- Assist with care plan meetings and follow-up activities.
- Help coordinate appointments, referrals, and community resources.
- Assist with discharge planning and transition-related activities.
- Support resident rights, dignity, privacy, and quality of life.
- Participate in interdisciplinary team meetings and facility activities.
- Maintain confidentiality and follow all facility policies and applicable regulations.
- Previous experience in a healthcare, long-term care, social services, or customer-service environment preferred.
- Strong communication and interpersonal skills.
- Compassionate and patient approach when working with residents and families.
- Excellent organization and attention to detail.
- Ability to work effectively as part of an interdisciplinary team.
- Proficiency with basic computer applications and documentation.
- High school diploma or equivalent required; additional education in social services, human services, psychology, or a related field preferred.
