** About the CAP Program
** CAP provides Intensive supportive housing for adults with severe and persistent mental illness. Clients receive medically necessary on-site services in a clustered apartment setting because of a history of failure in multiple community settings and/or the presence on an ongoing risk of harm to self or others which would otherwise require long term psychiatric hospitalization.
** What You'll Do
*** Monitors clients in a safe environment, performing and documenting client safety rounds. This includes ADL monitoring and coaching (i.e. life skills, communication skills, community integration, etc.).
* Facilitates structured activities with clients in accordance with their goals and treatment plan.
* Demonstrates culturally competent respect for all clients, guests and staff through verbal and non-verbal behaviors and assists them as needed.
* Maintains cleanliness of the program space.
* Safeguards Protected Health Information (written and electronically) and maintains confidentiality. This may include providing written documentation, scanning and/or uploading records into the Electronic Medical Record (EMR) system. This may also include requesting and/or releasing client information or medical records.
** Requirements of the Position
*** Availability to work a ENTER schedule (40 hours per week) Mon-Fri 8A-4:30P; all new hires are required to attend a
** one-week ReDiscover orientation
** as part of the onboarding process.
* High School Diploma or GED.
* A valid Missouri Class E automobile operator’s license (or equivalent license in another state).
* Must carry all appropriate insurance on the vehicle being driven, necessary to comply with Missouri or Kansas State Law.
* Working knowledge of mental illness and substance use disorders preferred.
* Excellent customer service skills with a recovery focus.
* Strong written, oral and interpersonal communication skills.
* Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as the use of email.
