The Intake Coordinator is responsible for the facilitation of requests for services, resource referral, assessment of patient/family clinical needs, and interfaces with managed care organizations, external reviewers, and other payers.

Job Duties /Responsibilities

Participates in patient admission procedure, vital signs, obtains lab specimens, assists in patient ADL’s, preps patients for diagnostic exams, checks belongings for contraband, and all patient care procedures.
Ensures patient safety by identifying and reporting to an RN any overt and covert potential for purposeful and/or accidental self-injury.
Demonstrates basic knowledge of emotional and mental health disease concepts,
Completes all required documentation in a clear, concise manner, showing understanding of patient goals and treatment plan.

Qualifications

Job Requirements:

Graduate Degree in social work, mental health/behavioral sciences, or related field and/or
Currently licensed to practice as a Registered Nurse (RN) by the state of Kentucky
Ability to read, analyze and interpret common scientific and technical journals, patient evaluations and information reports
Ability to respond to common inquiries or complaints from customers, regulatory agencies or members of the community
Ability to communicate on an effective level with physicians, patients, referral sources, and staff from a variety of disciplines
Completion of CPR course and completion of HWC within 90 days of employment
