Office Nurse​/LVN

We are in need of a LVN to work in office 3 days a week; part time as intake backup as well as Clinical Care Coordinator duties. The duties include the following:

• Intake: assist with the evaluation of incoming referrals for appropriateness for admission. Completes intake information sheet on every pending referral. Accurately enters patient information into EMR.
• CCC:
  Assists intake and OASIS team of compliant F2F documentation from SOCs. Assists with the evaluation of pertinent clinical information from referral sources to ensure agency is able to meet the needs of the patient. Obtains clinical report from RN on all SOC, ROC and RC OASIS visits and monitors/facilitates effective RN communication regarding plan of care.
