Community Health Worker; Navigator

Provide culturally appropriate outreach, enrollment, and navigation services to anyone eligible for the Healthier Oregon Program.

• Provide participants with enrollment, redetermination, and navigation services throughout Clackamas, Marion, Multnomah, Polk, and Washington counties.
• Attend outreach events and track and document the number of individuals reached.
• Follow up with participants in order to assist them with ongoing system navigation.

• Bilingual, bicultural English and Spanish is required.
• A high school diploma or equivalent is required.
• Must be a Certified OHA Assister or become certified within 6 months of taking the position.
• Have a Community Health Worker certification or become certified within 6 months of taking the position.

Prior to a new hire, a background check, including criminal record history, will be conducted. Information from the background check will not necessarily preclude employment.

This position requires the use of a personal vehicle and a minimum level of insurance coverage for that vehicle.

Personal liability insurance with $50,000 for bodily injury per person, with an aggregate of $100,000 per accident, or

$100,000 combined single limit (bodily injury and property damage combined)
