** Responsibilities
* ** Provide comprehensive patient care through assessment, diagnosis, and treatment planning in home health settings.
* Develop and implement individualized care plans, focusing on rehabilitation and mobility enhancement.
* Educate patients and families on physical therapy techniques and home exercises.
* Collaborate with interdisciplinary teams to ensure continuity of care and effective discharge planning.
* Maintain accurate documentation of patient progress and treatment outcomes while adhering to medical terminology standards.
* Stay current with best practices in physical therapy, particularly in hospice and geriatrics.

* Qualifications:

*** Licensed Physical Therapist in Washington state (or before your first day of work)
* One to two years of experience as a Physical Therapist
* Current BLS certification
