Banner Behavioral Health has 156 licensed inpatient beds and is located in Scottsdale, AZ. Our focus is on adults and adolescents dealing with mental health and/or chemical dependency issues who voluntarily seek help. POSITION SUMMARY This position provides comprehensive care coordination for patients as assigned. This position assesses the patient’s plan of care and develops, implements, monitors and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patient’s health care needs. The intensity of care coordination provided is situational and appropriate based on patient need and payer requirements.

This position is accountable for the quality of clinical services delivered by both them and others and identifies/resolves barriers which may hinder effective patient care.  CORE FUNCTIONS
1. Manages individual patients across the health care continuum to achieve the optimal clinical, financial, operational, and satisfaction outcomes.  
2. Provides assessment, direct psychoeducation, and direct psychotherapy to individuals, families, and groups. Coordinates all aspects of patient care from admission to discharge, interacting with the physician and other members of the clinical treatment team to provide quality patient care.
