Park DuValle Community Health Center is seeking a dedicated and compassionate Community Healthcare Worker to join our team. The ideal candidate will play a vital role in promoting health and wellness within the community by providing support, education, and resources to individuals and families. This position requires strong interpersonal skills and a commitment to improving the health outcomes of diverse populations.
Key Responsibilities- Provide health education and resources to community members.
- Assist individuals in accessing healthcare services and navigating the healthcare system.
- Conduct outreach activities to promote health initiatives and programs.
- Collaborate with healthcare professionals and community organizations to address health disparities.
- Monitor and report on community health trends and needs.
- Support patients in managing chronic conditions and adhering to treatment plans.
- Facilitate workshops and support groups to enhance community health awareness.
- High school diploma or equivalent; relevant certification or degree in healthcare or social services preferred.
- Strong communication and interpersonal skills.
- Ability to work effectively with diverse populations.
- Knowledge of community resources and healthcare services.
- Experience in health education or community outreach is a plus.
- Strong organizational and time management skills.
- Compassionate and empathetic approach to patient care.
