**** Home Health Aides – Full Time
**** 2nd

Shift: 3:00 PM - 10:30 PM (Every Other Weekend)

* Qualifications:

*** High school diploma or GED required
* HHA Certification, STNA/CNA preferred but not required
* Valid driver’s license, automobile insurance, and reliable transportation

* Key Responsibilities:

*** Provide personal care and assistance with daily living activities (bathing, grooming, oral hygiene, dressing, toileting, skin care, etc.).
* Document care provided and report changes in the client’s condition (physical, emotional, or medical) to management.
* Take and record vital signs as instructed.
* Prepare and serve nutritious meals.
* Support mobility through ambulation, range-of-motion exercises, and activity assistance as outlined in the care plan.
* Assist with clients with medication reminders.
* Maintain a clean and safe environment: change linens, do laundry, provide light housekeeping and pet care assistance.
* Ensure all equipment is used safely and all care is provided in compliance with safety standards.
* Complete and submit documentation accurately and on time.
