Duties/Responsibilities:- Develop relationships and collaborate with the primary care teams to conduct social services assessments to determine the appropriate needs for each member
- Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers
- Navigatemanagedcareplansforcommunityservicesandprograms
- Casemanagementforsocialandbehavioralcaretoallowmemberstoself-managehealthandsocialservicesupport
- Accurate and timely documentation of patient encounters and sessionsinallclinicalmanagementsystems
- Proactively identify methods toimprove Arch Well Health 's approach based onfeedbackandregularlyconductedsurveys
- Support advance care transitions with members and their families
- Assist members with access to state-based prescription programs and other benefits
- Experiencesupportingpatientswithneedforsocialservices
- Experiencewithscreening,assessment,andplanningforcommonsocialservicesneeds
- Workingknowledgeofdifferentialdiagnosisofcommonmentalhealthconditions
- Stronginterpersonalcommunicationskillswithexceptionalactivelisteningabilities
- Highly empathetic,non-judgmental,andopen-minded
- Experienceinacollaborativeteamenvironment
- Fully licensed LMSW in the desired State of practice
- Master'sdegreein
