Medical Social Worker; MSW

Renew Home Health is looking for a compassionate Medical Social Worker (MSW) to support our patients and their families where they live. In this PRN role, you help patients navigate the personal and practical side of their care — connecting them with resources, easing transitions, and making sure no one falls through the cracks. • Assess each patient’s psychosocial needs and the social factors affecting their health and recovery
• Connect patients and families with community resources — financial assistance, transportation, meals, caregiver support, and long-term care options
• Provide short-term counseling and emotional support
• Help with advance directives, care planning, and safe transitions of care
• Coordinate with the nursing and therapy team on each patient’s plan of care
• Document each visit accurately using our AI-assisted charting tools

• Master’s degree in Social Work (MSW) from a CSWE-accredited program
• Current Texas social work license (LMSW or LCSW)
• At least one year of social work experience in a healthcare setting (home health experience a plus)
• A valid Texas driver’s license, reliable transportation, and current auto insurance
• Strong communication skills and a caring, patient-first attitude
