Part-Time Patient Care Coordinator (PCC) – Dutchess County, NY

Visiting Docs is seeking a dependable and organized Part-Time Patient Care Coordinator (PCC) to provide specialty coverage at various healthcare facilities throughout Dutchess County, NY
. Coverage areas may include Poughkeepsie, Wappingers Falls, Beacon, and surrounding areas
.

Schedule:

Part-time, 2–3 days per week
Approximately 16–24 hours per week
Shifts scheduled between 9:00 AM and 5:00 PM
Monday through Friday; specific days may vary based on coverage needs

Position Overview:

The PCC will travel among assigned Dutchess County facilities to provide administrative and patient coordination support for our specialty services. This position is ideal for someone who is flexible, self-motivated, organized, and comfortable working independently at multiple locations.

Responsibilities may include:

Supporting specialty providers at assigned facilities
Coordinating patient schedules and appointments
Communicating with facility staff, providers, and the Visiting Docs team
Assisting with patient documentation and administrative needs
Ensuring information is accurately communicated and appropriately followed up
Providing coverage at different Dutchess County facilities as needed

Requirements:

Reliable personal transportation and a valid driver’s license
Ability and willingness to travel between facilities throughout Dutchess County
Strong organizational and communication skills
Professional, dependable, and comfortable working independently
Prior experience in a healthcare, medical office, or patient coordination setting preferred
