Position Summary

This is a FT hybrid position based in Portland. This role collaborates with the Director of Population Health, Health Equity and Quality, plan staff, communities, healthcare providers, pharmacies, prescribers, and other partners to advance health equity. The position focuses on pharmacy‑related health equity and quality improvement, facilitating culture change and addressing patient experience, outcomes, access to care, health related barriers and social needs.

It also supports efforts to reduce health inequities caused by factors such as racism, poverty, ineffective policies, and systemic obstacles.

Pay Range

$65,682.16 - $82,107.97annually (depending on experience). This role may be classified as hourly (non‑exempt) depending on the applicant’s location.
* Actual pay is based on qualifications. Applicants who do not exceed the minimum qualifications will only be eligible for the low end of the pay range.

Required Skills, Experience & Education

Bachelor's degree in Public Health or an equivalent combination of education and relevant work experience.

2 years of relevant experience including working in or with rural or underserved communities, working with leadership, facilitating meetings, measuring, and monitoring programs evaluation activities and working with multi‑disciplinary, multi‑cultural teams.

Equity advocacy or system change efforts experience preferred.

Document and report quality initiatives to appropriate internal and external stakeholders.

Through healthcare research and quantitative analysis, summarize and synthesize complex data and information to identify trends and patterns, pinpoint issues, and areas of improvement, and recommend solutions and next steps.

Coordinate with marketing staff and vendors as needed to update, create, or purchase relevant materials and communications.

Work with other departments to develop and vet program materials using guidelines established by state partners and the department.

Ensure compliance with applicable state and national quality assurance and quality improvement standards.

Develop annual project work plan resource allocations and budget impacts.

Oversee data collection and analysis of data and prepares pertinent documentation and reporting for quality improvement activities, including queries and member and provider surveys.

Monitor and report the outcome of health promotion and quality improvement committees, regulatory agencies, and other oversight entities.

Develop and maintain quality improvement policies and processes.

Review pharmacy policies and procedures on an annual basis to address health inequities.

Assist with pharmacy health promotion activities to improve the quality and safety of clinical care programs/services provided to pharmacy members.
